Novel Strategies for Endovascular Treatment of Abdominal Aortic Aneurysms Based on Immune Microenvironment Regulation

Abstract

Abdominal aortic aneurysm (AAA) represents a significant cardiovascular pathology characterized by progressive aortic dilation and a high risk of fatal rupture. While traditional endovascular aneurysm repair has revolutionized AAA management by providing mechanical flow isolation, it often fails to address the underlying biological drivers of the disease, leading to long-term complications such as endoleaks and continued sac expansion. To overcome these limitations, this study proposes a novel conceptual framework for integrating immune microenvironment regulation into the endovascular treatment of AAA. The proposed strategy aims to complement the traditional mechanical flow isolation paradigm by developing advanced immune-modulatory endovascular devices. These innovative devices are specifically designed to modulate local inflammatory responses, guide macrophage polarization and immune cell behavior, and promote favorable vascular tissue remodeling at the aneurysm site. The comprehensive framework will be rigorously evaluated through a multi-stage preclinical evaluation pipeline, encompassing detailed in vitro cellular experiments, robust in vivo animal models, and advanced computational fluid dynamics simulations. Ultimately, the proposed framework may provide a rational and highly effective translational roadmap for the development and preclinical evaluation of next-generation immune-modulatory endovascular devices. By directly addressing the immune-driven pathological mechanisms inherent in AAA progression, this interdisciplinary approach holds substantial promise for significantly improving long-term therapeutic durability, enhancing patient outcomes, and establishing a new standard of care in vascular surgery.
